By registering your child(ren) and/or youth(s) through e-mail for our program(s) and membership, you agree and consent to the following program waiver and member consent:
PROGRAM WAIVER
For my son(s)/daughter(s), myself, all our respective heirs, executors, administrators and assigns hereby release, waive, and discharge the GLOUCESTER RECREATION DEVELOPMENT ORGANIZATION (GRDO) (the Board), its servants, agents, instructors, and employees of and from any claims, demands, damages, actions or causes of actions arising out of or in consequence of any loss, harm, or injury, to the child(ren) identified above, or damage to his/her/their property, incurred while attending at, participating in, or going to and from any activity, program, trip, or excursion organized by the Board (an “Activity”), notwithstanding any such loss, harm, injury, or damage may have arisen or been contributed to be reason of negligence of the Board, its servants, agents, instructors, or employees. I further undertake to hold and save harmless and agree to indemnify all of the aforesaid from and against any and all liability whatsoever incurred by any or all of them arising as a result of, or in any way connected with the attendance and participation by my said son(s)/daughter(s) in an Activity.
MEMBER CONSENT
In becoming a member of the Gloucester Recreation development Organization (GRDO), my child(ren) and/or youth(s), and I agree to abide by the rules of conduct and procedures of the organization, including at program(s) and at tournaments. I understand that my membership is valid for the entire CURRENT CALENDAR YEAR in which I am registering. The family membership entitles all eligible family members to participate in the activities organized by the GRDO during the current calendar year. I give the Gloucester Recreation Development Organization permission to take my child(ren)’s photograph or video which may be used on the GRDO’s public website, in print, electronic media and/or community newspapers for the promotion of our programs and services.
ELECTRONIC COMMUNICATIONS
In becoming a member of the Gloucester Recreation development Organization (GRDO) and by registering for our program(s), you agree and provide consent to receive electronic communication via email, including messages regarding programs, and other relevant and important information regarding our programs. You will have the opportunity to opt-out of receiving our emails each time we contact you. During programs which you are registered for, GRDO will send real-time text-alert messages to the mobile number(s) provided during registration which include reminders on program start and end dates, cancellation notices, and important and relevant event information. You will have the opportunity to confirm and provide consent on the initial text-alert you receive.
